CASE STUDIES
BPPV
*John had a 3 week history of intermittent dizziness and room spinning sensations associated with nausea. It started when he woke up in the middle of the night after rolling over which triggered a sudden onset of nausea and a room spinning sensation. He managed to fall back asleep and since then he has noticed the symptoms return when he lies down, gets up from bed, or if he looks up or down. He also describes a constant fogginess in his head which he describes a slight drunk sensation.
Assessment by the physiotherapist revealed that John was suffering from a very treatable condition known as benign paroxysmal positional vertigo (BPPV). The physio a few treatment techniques which cleared the vertigo within a few days.
Vestibular Neuritis
*Sarah a 45 year old female, developed a very sudden onset of nausea, intermittent vomiting, dizziness, and constant room spinning sensations. This lasted 2-3 days. After the initial 2-3 days, Sarah’s symptoms have slowly improved over 3-4 weeks where she now experiences light-headedness and describes a “delayed and foggy” brain which is typically worsened but quick head movements or postural changes.
After she was assessed by a vestibular physiotherapist, she was diagnosed with a possible vestibular neuritis which caused by an infection which attacks the inner ear nerve.
Sarah was assigned some exercises to do to retrain her brain and the nerve to work more efficiently. Sarah’s symptoms gradually improved over a further 3-4 weeks and now feels close to normal again.
Vestibular Migraines
*Lisa a 29 year old female, slowly developed symptoms of dizziness which she described as a feeling of being hungover all the time. She was also experiencing moments of sensitivity to light and sound and room spinning sensations.
Her symptoms seemed to fluctuate in severity and was some good days and bad days. Lisa had just started a new job in a very busy accounting firm after moving interstate a few weeks before her onset of symptoms.
In the 8 months that she has had these symptoms, she had seen multiple doctors and therapists to try and get answers but nothing seemed to help. She was prescribed many different types of medications but none helped to a large effect.
She finally found a vestibular physiotherapist who did a thorough assessment which lead to the diagnosis of vestibular migraine. She had a very detailed explanation as to what this meant and why she was feeling dizzy. Together they made some slight lifestyle modifications to decrease the triggers and was given exercises to improve her inner ear function and balance.
